Guidelines
for best patient and lens selection to optimize success with
the MagniVue MF lens:
Begin with a full examination which should include a comprehensive
patient history and details of their visual requirements and needs,
along with the normal slit lamp evaluation and keratometry refraction.
In addition measure amplitudes of accommodation, working distance
and determined eye dominance.
Best
Vision Sphere:
Give the maximum PLUS that will maintain good visual acuity. Astigmatism
of up to 0.50DS is usually acceptable.
Ophthalmoscopic and slit lamp examination should help to rule out
any contraindications to wearing soft lenses. The lens is successfully
fitted on a wide range of corneal shapes with only one base curve.
Insert
the lens and immediately check the acuity. This should be very
close to the final visual acuity.
Wait one hour and do a complete
examination. There should be movement of 1.00-1.50mm. A static lens
is not acceptable. Your over refraction should be up to ±0.50DS
and then adjust accordingly. Short sighted patients – leave
on red.
Fitting
MagniVue MF lenses:
By
following the outlined steps, fitting the MagniVue MF lens is
easy. To optimize success, it is important
to remember that the
fitter’s positive attitude is essential. The contact lens
specialist must be able to instill confidence in the patient. It
is important to have a large stock of lenses to choose from, enabling
the patient to leave the fitting session with a lens prescription
that is close to the optimal prescription. By taking notice of the
suggested fitting pearls, it should be possible for you to achieve
over a 70% success rate.
Choose
candidates carefully:
1. Evaluate vocational and personal needs appropriately. Educate
the patient regarding some possible compromise in vision. The distance
and near vision may not be as sharp as the visual acuity achieved
when wearing spectacles or single vision contact lenses, but it
should be comparable.
2. Ensure current distance and near subjective refractions are
accurate and keratometry readings will be necessary for selecting
the initial lenses.
3.
Contact the patient three to four days after dispensing the lenses.
If the patient is unhappy with their
vision, then advise
them to discontinue wearing the contact lenses until his next visit
to the office, when you can address the problem properly. However,
ask the patient to wear the lenses at least four hours before they
come for the recheck visit. This procedure is important to ensure
that the patient doesn't just give up and decide prematurely
against wearing multifocal contact lenses.
IMPORTANT
TIPS TO REMEMBER:
1. Increase your success rate by selecting carefully possible candidates
to fit.
2. Always check the patient binocularly with their multi focal
contact lenses.
3. Check the reading acuity with good illumination at a distance
of 40cm.
4.
Over refraction binocularly in ±0.25DS
steps and change accordingly.
5. Check the patient for distance using an outside vista. Remember
the reading zone is in the centre of the lens.
6. Hypermetropes will adjust to near vision immediately. Distance
vision will improve in three to seven days.
7. Do not over refract the MagniVue MF lens using an auto refractor.
Fitting:
1. Slit lamp evaluation and routine checks for contact lenses
2. Accurate current distance and near subjective refractions
3.
K readings – remember the lens has
one base curve of 8.70mm. Select the average K reading between
7.40 to 8.00
4. Selecting the first choice lenses should be based on the following:
